Veterans

The men and women of our Armed Forces make incredible sacrifices in service to our country, and the natural, drug- and surgery-free approach of chiropractic care can be especially beneficial to active duty military and Veterans. The MAC’s Veterans Task Force has developed a toolkit to help you navigate the maze that is the VA Health System and begin caring for your community’s veterans.

Battle Creek VAMC Information:

Counties included: Mason, Lake, Oceana, Newaygo, Mecosta, Muskegon, Montcalm, Ottawa, Kent, Ionia, Clinton, Allegan, Barry, Eaton, Ingham, Van Buren, Kalamazoo, Calhoun, Berrien, Cass, St. Joseph, Branch. 

If you have an office in any of these counties, and would like to be eligible to receive referrals from the Battle Creek VAMC, you must fill out the following forms:

Upon completing these forms, please email scanned copies to Mr. Shawn Dougherty at shawn.dougherty@va.gov, OR fax to (269) 223-6254.  When possible, please scan and email the documents – this will better insure that the documents have been received by Battle Creek VAMC.

Counties included: Washtenaw, Hillsdale, Lenawee, Monroe, Jackson, Livingston, Shiawassee.

If you have an office in any of these counties, and would like to be eligible to receive referrals from the Battle Creek VAMC, you must fill out the following forms:

Saginaw VAMC Information:

If you are interested in providing chiropractic care to Veterans in your community, complete the “Provider Agreement Packet” and “VCP Checklist” below. If you have any questions regarding the provider agreement paperwork, contact Michelle Sorie at the Saginaw VA’s Office of Community Care at (989) 497-2500, Ext. 11501, or michelle.sorie@va.gov. The following documents will be needed: 

  • Provider Agreement Packet
  • VCP Checklist
  • Liability Insurance Documentation
  • For a larger group practice, the practice credentialing policy
  • If no credentialing policy, a copy of license to practice chiropractic 

Upon completion, the documents should be sent to Ms. Sorie by fax at (989) 321-4971 or scanned and emailed to michelle.sorie@va.gov.
